Krypton: Vocalist = Singer. ER = common indication of hesitation in direct speech. As in "Er.. mm.. are you sure?" Vocalist without hesitation = singer - er = SING. Healthy = well. Internally is containment indicator. So we have (Singer -ER) <--- WELL = S WELL ING= unhealthy enlargement. Strictly speaking the unhealthy is not needed, swelling is enlgargement. But it was added by the setter to make the clue read a little funny and to misdirect the solvers.
